Given that OnChange doesn't trigger over FTP, it must be changes on your local side that actually trigger the scan. But the scan - once triggered - may pick up files which are different on either side. These may include files on the FTP side which are in mid-write, but your FTP server should deny access to them (refuse to allow them to be copied) if so. If it doesn't, you may get incomplete copies downloaded, yes.
